How the Plan Works
An “indemnity” plan means you are reimbursed for a medical service based on the plan chosen (click here for Schedule of Benefits and Rates). For example, if you were to be hospitalized and the cost was $800 per day and the reimbursement limit for the plan you chose was $1,000 per day, the $800 cost would be covered 100%. Whereas, if the hospital cost was $1,100 per day, the plan would reimburse up to the $1,000 per day limit, so there would be an out-of-pocket expense of $100.

         monthly for a family. Annually, that amounts to approximately
         $1,080 for an individual and only $2,616 for a family.

How You Get Reimbursed

• When you visit an in-network provider for an office visit, you pay a "copay" for the visit in the amount of the Plan you selected. It's that simple. No Claim Form to fill out.
• If you visit an out-of-network provider you pay for the service at the time of the visit. Then, you submit a Claim Form along with a copy of the doctor/hospital bill and a reimbursement check is mailed to you in the amount of your bill up to the coverage provided by the Plan you selected. Reimbursement checks are usually processed within fourteen (14) business days.
